All personnel involves in services for children and youth under the age of 18 must complete and submit this form. It will be received by the Parish Priest of your community.
I have read, accepted, and understood the Catholic Diocese of Hong Kong’s " Guidelines for the Protection of Children and Vulnerable Persons" and I agree to contribute on this premise to establish a safe church, protecting and respecting all individuals, especially children and vulnerable persons.
